Around here (south Florida) you can get blank proposal forms from Office Depot, Office Max, Staples, etc. Regarding Inspection forms, I do inspections for Citizen's Insurance and they provide the form. I also do inspections for a number of realtors and banks in which case they either provide the form or I just type up a letter and describe the condition of the roof. Estimating forms can also be picked up at a stationary store. I just use a little plain paper notebook for my notes from the site visit. Then take my notes and write my proposal. Regarding the warranties, each manufacturer general has a warranty form that you can get either from the manufacturer or distributor (or their respective websites).
